Independent Living Skills Score List TEMPLATE

This is a tool to measure independent living skills and strategies key work sessions to ensure complete independence before the resident moves out of supported accommodation. This builds evidence when asking for additional support hours or why building a case why a resident needs to remain in supported accommodation. 

It is a useful document for Residential Children's Homes to complete with a young person, too, before their 16th birthday or before they move out of the home.
